What do you say when someone tells you they're pregnant?

It feels extremely disingenuous to congratulate someone when they announce a pregnancy. Or when a friend tells me another friend is pregnant or in labor, etc. My standard response is gagging, but I don't feel I can always get away with that.

So what do you do?
